On October 5, 2011, at approximately 1:00 p.m., Juan-Hernandez Rosas was killed in a multi vehicle crash while working as a traffic flagman for a construction crew installing a beam on I-465 over 30th Street in Indianapolis, Indiana.

At the time of the accident it has been reported that Mr. Rosas was holding a “slow” sign in the roadway. He was hit by a Hyundai, and then multiple other vehicles were also involved in the crash.

It has further been reported that the occupants of the Hyundai were ejected from the vehicle by the impact of the collision.

Mr. Rosas was 43 at the time of his death.
